I think I have found the issue.
The planar orientation differs in the .hdr files.

Subject: Importing a Segmentation
I would like to use Seg3D to edit a binary mask that is created outside of
Seg3D. I have attempted the following:
File - Open - I open the MRI volume
File - Import Segmentation - I specify the binary mask
The binary mask and MRI both have same voxel dimensions and datatype. When the
segmentation is imported, it seems to be in a different space (ie not is the
same space as MRI).
I am using Seg3D 1.12.0
