Lincoln,

I'm sure I could help with this, but it depends a lot on how much of the tooth/filling has been segmented.  Also, as I'm not a dentist I'm not sure what part of the filling would be considered the isthmus; looking top-down on the tooth, could you find the first slice containing canal but no isthmus in the current segmentation and clear it (using the paint brush tool on the filling mask and pressing "E") and then run a connected component filter on the isthmus to extract it?  Having a model to examine would make solving this a lot easier.
